Health booklet and medical appointments

The attention given to the policy-holder is registered in this document; subsequent appointments are scheduled in each of the services responsible for taking care of him.

Recommendations:

• Come to the appointments on the day and time scheduled in the booklet.
• In case of emergency come to the Service of Continuous Attention in your Family Medical Unit or to the Emergency Service of your appointed Hospital.

For your son's health, follow carefully all the instructions from your doctor, nurse or health staff who currently take care of him.
